You’re Not Losing It, You’re Waking Up to a New Phase of Health

For a lot of women, perimenopause symptoms do not arrive with fireworks and hot flashes. It slips in quietly, through restless nights, unpredictable moods, brain fog that feels suspiciously like early-onset dementia, and a sudden intolerance to stress that used to roll off your back. Your go-to strategies, clean eating, workouts, supplements, maybe even ADHD meds, suddenly stop delivering results. You start to wonder if you’re doing something wrong.

You’re not.

What you’re feeling is not random. It’s not your fault. And it’s definitely not “just aging.” It’s a physiological shift with very real implications. The problem is, most women don’t get the memo early enough. Perimenopause can last 4–10 years, and if we only start paying attention when we’re drenched in sweat at 3 a.m., we’re already behind.

The earlier we start paying attention to what our bodies are telling us, the more power we have to change the trajectory.

Most Perimenopause Symptoms Aren’t the Start, They’re a Signal You’ve Already Entered Perimenopause

Mood swings, irritability, anxiety, and poor sleep are often chalked up to stress or being “busy.” But declining progesterone and fluctuating estrogen levels directly impact our nervous systems, our sleep architecture, and yes even how we respond to ADHD medications.

If you’ve been diagnosed with ADHD, you might notice your meds seem less effective. That’s not in your head estrogen plays a role in how our brains process dopamine and norepinephrine, which are key to ADHD management. As hormones shift, many women feel like their usual treatment plans need constant adjusting. That’s normal, but it’s not often discussed.

Then there’s cortisol, on overdrive in this phase, which adds fuel to the fire. Not only does it disrupt sleep and focus, but it also drives belly fat storage and keeps the body in a stressed, inflamed state. You’re not imagining the weight gain or the fact that your old meal plan and workout routine no longer cut it.

HRT Isn’t Just for Symptoms, It’s Preventive Care

There’s a misconception that hormone therapy should be a “last resort.” That it’s just for when symptoms are unbearable. But hormone replacement when used appropriately and safely can act as preventive care. We’re talking about protecting bone density, cardiovascular health, cognitive function, and overall quality of life. Yes, HRT often improves symptoms dramatically, but the goal isn’t just to “fix” you. The goal is to support you.

Still, hormones are just one piece of the puzzle.

Nutrition and Protein First: What Actually Moves the Needle

If you’re already paying attention to what you eat and how you move and still not seeing results, it’s not about willpower. It’s about physiology. What used to work often stops working in this phase of life. That’s because the hormonal environment has changed, and so must your strategy.

I don’t love the word “diet.” I’m not interested in short-term fixes or calorie math. What I do believe in is using food as a tool, starting with protein first, then layering in fiber-rich carbs like vegetables, lentils, and berries. From what I see in practice, this order of operations supports blood sugar balance, reduces cravings, and helps rebuild lean muscle mass, which declines faster in this stage.

Sometimes we also use tools like lab testing or CGMs (continuous glucose monitors). For some, that may feel overwhelming. But for others, especially those who are already diligent, it’s the missing information they need to get unstuck. When used strategically, these tools can help identify where things are going off track before more serious issues take hold.

You Don’t Have to Figure This Out Alone

Perimenopause is not a problem to solve, it’s a transition to navigate with intention. At The Clinic, I work with women one-on-one to help decode their symptoms, personalize treatment options (including HRT when appropriate), and support their health goals with real-world strategies that fit their lives.
